Here are some reminders-

• Clients with Cardiovascular Disease, Diabetes or Chronic Heart Failure can still enroll into our Embrace CSNP regardless of what type of plan they have. Even beneficiaries with Original Medicare Only or a MedSup can switch to a CSNP Plan right now!

• Beneficiaries with LIS or Medi-Cal can change right now using their once per quarter SEP
